Approving the request and sending it to the beneficiary
Special conditions
- The applicant must be registered & licensed
- The beneficiary pays fees: 5 Omani riyals for non -Omanis and free for the Omanis
- Minimum of 6 months' working experience in the same institution
- The form is to be completed by the applicant's direct manager. never be completed by the applicant
- There was not any disciplinary action taken against the applicant
- The applicant has NOT committed any illegal offence
- There must not be any disciplinary action / court case pending against the applicant
- Signature, date, and stamp of the Director General/Hospital Director/or their representative: Mandatory
- Department/Institution Stamp: Mandatory
- Non-Conviction certificate from the ROP, for non-Omanis applying for end of service good conduct certificate
